PARENT SURVEY

 www.ESEsurvey.com

The Florida Department of Education (FDOE) requests the participation of parents with students in Exceptional Student Education in completing a survey designed to provide the state with information on how well schools are supporting parental involvement in their Child’s education.  Under the individuals with Disabilities Act (IDEA), each state is required to annually collect this information from parents of children receiving special education services and report it to the federal government.

This information only comes from parents; therefore, it is very important that a large number of parents participate in this effort.

Please go online to www.ESEsurvey.com anytime between now and June 30, 2010 to complete the survey.  Data is completely anonymous.  This confidential information is collected on a monthly basis.  If you do not have access to a computer, the paper survey can be obtained by contacting Ann Koebe, Parent Liaison at 219-1200 extension 30192 or contacting your school’s Mainstream Consultant or ESE contact.

For more information please contact Ann Koebe, Parent Liaison for Exceptional Student Education at 219-1200 extension 30192 or e-mail to koebea@martin.k12.fl.us.

We would like your feedback.  Please tell us what you think!
